Job Description

The terms of reference (TOR) establishes a guide to facility-based case manager (trained ad-hoc personnel) on duties and responsibilities, expected of them as they support in providing services within AHNi supported Health facilities across the 13 ICHSSA 2 project local government areas in Edo State. Facility-based case managers are to be domiciled within these Health Facilities, and all activities are to be carried out in partnership with AHNi. This TOR is also aimed at strengthening bi-directional (community and facility) epidemic control activities between ARFH and AHNi. This TOR highlights roles of the facility case managers across three (3) key areas as outlined below:

  1. The conduct of folder Audit: to elicit and track treatment services yet to be received (E.g Viral and drug refill appointments) and those not received (Missed appointments, LTFU) by the beneficiaries, for Community level follow up.
  2. Facilitate all facility level program support
  3. Provide Documentation support

 Summary

The facility case managers, will be responsible for facilitating HIV Treatment and Care across the 95-95-95 cascade at the facility for all ARFH ICHSSA project beneficiaries (Children 0-17years and Caregivers >18years) who are HIV positive and accessing treatment across all AHNi supported health facilities located in the 13 project LGAs outlined above.

The Facility Case Manager recruited for this position will have his/her primary place of assignment at the AHNi supported facility (ies) and will work in close collaboration with AHNi case managers to ensure the achievement of the 95-95-95 targets at the designated facility (ies).

Specific Responsibilities

The facility case managers, will be responsible for facilitating HIV Treatment and Care across the 95-95-95 cascade at the facility for all ARFH ICHSSA project beneficiaries (Children 0-17years and Caregivers >18years) who are HIV positive and accessing treatment across all AHNi supported health facilities located in the 13 project LGAs outlined above.

The Facility Case Manager recruited for this position will have his/her primary place of assignment at the AHNi supported facility (ies) and will work in close collaboration with AHNi case managers to ensure the achievement of the 95-95-95 targets at the designated facility (ies).

 

  1. Conduct Monthly folder audit of beneficiaries on treatment in the facility.
  2. Documentation of indicators on the Combined Monthly Template (CMT) and High Frequency reporting.
  3. Facility Program Support.
  • Support OTZ Club meetings and enrollment of adolescents.
  • Support Adult facility support group and enrollment of adolescents.
  • Support HIV Case finding at all service delivery points.
  • Support Facility – Community referral with proper documentation at both facility and CBO level.
  • Support appointment reminder for drug pick-up and viral load sample collection.
  • Support client tracking for defaulters and lost to follow-up.
  • Support Index case testing/Genealogy elicitation on ART clinic days.
  • Support Referral Coordination Meetings.

Required Experience

  • Applicant can be a Male or Female.
  • Should be resident within the LGA and around the supported facility(ies).
  • Passionate about providing care and support for children and Adults living with HIV.
  • Passionate about working with and being a mentor to children.
  • Have strong leadership qualities (Critical thinking, Humility, Honesty, Determination, Flexibility).
  • MUST be available full time and committed to the project.
  • Good listener with demonstrated proficiency for communicating in English, local languages of target LGA/community and pidgin).
  • Excellent skills in documentation and use of data tools.

 

Working relationship: Community Case Managers, CBO staff, Program monitor, AHNi FCMs, ARFH SI and Program staff.

Minimum Qualifications

  • Minimum of OND in science or social sciences.
  • Must have worked on HIV treatment and Care at the facility.
  • Previously worked with AHNi is an added advantage.
  • Age 19+ (Ideally between 20 – 35).
  • At least 1 -2 years’ experience of working in the field of OVC, HIV treatment.
